| Grade 3 | 0.5–0.99 | Severe neutropenia | Hold chemotherapy. Consider G-CSF. Avoid live vaccines. |
| Grade 4 | < 0.5 | Agranulocytosis / Life-threatening | Hospitalise if febrile. Broad-spectrum antibiotics. G-CSF. Protective isolation. |
Febrile Neutropenia: ANC < 0.5 (or expected to fall < 0.5) + fever ≥ 38.3°C. A medical emergency requiring immediate broad-spectrum IV antibiotics (piperacillin-tazobactam or cefepime) within 60 minutes of presentation.
