Normal ≥ 2.0 ×10³/µL Normal range No restrictions. Proceed with planned therapy.
Grade 1 1.5–1.99 Mild neutropenia Monitor closely. Most chemo protocols proceed with caution.
Grade 2 1.0–1.49 Moderate neutropenia Many protocols hold or dose-reduce. Infection precautions.
Grade 3 0.5–0.99 Severe neutropenia Hold chemotherapy. Consider G-CSF. Avoid live vaccines.
Grade 4 < 0.5 Agranulocytosis / Life-threatening Hospitalise if febrile. Broad-spectrum antibiotics. G-CSF. Protective isolation.
Febrile Neutropenia: ANC < 0.5 (or expected to fall < 0.5) + fever ≥ 38.3°C. A medical emergency requiring immediate broad-spectrum IV antibiotics (piperacillin-tazobactam or cefepime) within 60 minutes of presentation.
